Privacy Policy
This policy explains how the marketing division of Shak Web Group handles information collected on laimplantdentures.com.
What we collect
The answers you choose in the form, your name, phone, email and ZIP code, the page and campaign that brought you here, your IP address and browser type. We do not collect Social Security numbers, insurance member IDs or payment details.
How we use and share it
We may share or sell your request to one participating dental practice so it can contact you (the practice pays a flat fee per qualified inquiry, which can be a "sale" under California law), and keep a copy to confirm delivery, prevent fraud and handle billing with that practice. We do not sell your information to multiple buyers or to data brokers.
Sensitive information
Your answers describe your dental situation. We use them only to route your request and do not use them for advertising profiles.
Your choices
California residents can ask to know, correct or delete their information, and to limit use of sensitive information, by contacting [email protected]. Reply STOP to any text to stop texts.
Do Not Sell or Share My Personal Information
To opt out of the sale or sharing of your personal information, email [email protected] with the subject "Do Not Sell or Share" and the name, email or phone you used. We will stop selling or sharing your information within 15 business days. You can use an authorized agent to send the request.

Your rights
If you live in California you may ask us to: tell you what personal information we have about you and how we got it, used it and disclosed it (right to know); delete it; correct it; stop selling or sharing it (right to opt out); and limit the use of sensitive personal information. We will not discriminate against you for using these rights. We do not knowingly sell or share the personal information of anyone under 16. We will confirm your request within 10 business days and answer within 45 days. We verify requests by matching the name and email or phone you used. You may use an authorized agent; we may ask the agent for signed permission.
Send a request with the privacy request form or by email to [email protected].
Global Privacy Control and Do Not Track
We honor the Global Privacy Control (GPC) signal as a valid request to opt out of the sale and sharing of your personal information for that browser. When your browser sends GPC, this site does not load Google or Meta advertising or analytics tags, and any form request you send is marked "do not sell or share". We do not respond differently to the older "Do Not Track" browser setting, which has no agreed standard; use GPC instead.
Other California notices
Third parties (Google and Meta) may collect information about your activity on this site over time and across other websites when their tags load; they do not load when GPC is on. Under California Civil Code 1798.83 (Shine the Light) you may ask, once a year, which categories of personal information we disclosed to third parties for their direct marketing; email the address above. You can review the information you sent by asking us at the same address. When this policy changes we update the effective date at the top of this section and on this page.
Retention and security
We keep requests for up to 24 months for billing and dispute purposes, then delete them. Forms are protected by Cloudflare Turnstile and data is stored on access-controlled servers.
